REGISTRATION FORM
Please fill out the patient registration form before your consultation and bring it with you.
REFERRAL
Please bring a referral from your general practitioner. The referral needs to be dated within 12 months of your consultation.
IMAGING
Please bring all imaging to your consultation including x-rays, ultrasound, MRIs and CTs Scans.
INITIAL CONSULTATION
There will be an out-of-pocket expense for consultation in the rooms. A new patient consultation is $250 with a Medicare rebate available.
FOLLOW-UP
A follow-up review is $150 with a Medicare rebate available.
PROCEDURE
“There may be a known gap fee of $550 to $1100 depending on the procedure performed and your health fund benefits. Dr. Chandrasekaran’s staff will provide you with an itemised account for potential out-of-pocket costs and rebates for suggested surgical procedures.
If you elect to have your surgery in the public system, we can arrange this at no cost at Werribee Mercy Hospital, but there may be a waiting period for the procedure.
